As the episode opens, we see the Doctor all alone on the ship, with the rest of the crew apparently having abandoned ship. Then, we suddenly see B'Elanna enter the sick bay with the news that there's a ship wide emergency and the Doctor has to help treat everyone else.
The episode is interesting because it explores the complexity of the Doctor and we see him moving out of the sick by for the first time (even if it's actually a holodeck simulation, as we find out in the resolution). The episode moves his character forward, an evolution that will ultimately make him one of the most popular characters in the series. The episode also suffers from an abrupt resolution, a problem that afflicts many a Voyager episode.
